Standard Order Form (SOF) Information
Following is some information to help you use the SOF form.
To order educational materials on-line, use the "On-Line Catalog & Shopping Cart" available from any Educational Department web page, under the "Order Course Materials" pull-down menu.
To order other materials, complete the SOF using Adobe Reader version 7 or above, click the "Save Form" button at the bottom of the page to save your completed form to your computer, and then e-mail the saved file. Alternatively, you may postal mail or fax the completed hard copy form to USPS Headquarters at the address or phone number at the top of the form.

You can use either mouse or tab keys to move from field to field,
but use
of the tab key is recommended to ensure filling in all fields.

Start with the date field by clicking in the field area, or just
pressing
the Tab key when the form first opens. The date will
automatically
be entered.

Tab to the next field, and select or enter the requested
information.
Note that if using a credit card, some of the credit card data must
be selected from a pull-down menu using the mouse.

Use the space bar to select (check off) check box fields. A mouse
click can also be used.

If you hover your mouse over a field before clicking or tabbing to
that field, you will get a prompt that gives more information regarding
what is to be entered.

Tab through the remainder of the form in the same fashion to complete
filling
in all fields. You can use Shift+Tab to go back to the previous
field.

Check the data thoroughly, and then save a copy if you wish to re-use some or all of the data on the form. Use the "Save Form" button to save a copy to your computer.

Print a copy and check it thoroughly prior to sending it to Headquarters using either e-mail (if you saved a copy to your computer), fax or postal mail. Note that some computers will allow you to "print" a copy to your fax modem if you have one installed on your computer.
Click SOF to open the PDF form to send your
order
to Headquarters.
